Shifting of ECHS Polyclinic from CHAF, Bangalore to Permanent Location

A type A ECHS Polyclinic was functioning at CHAF, Bangalore since Jun 2004.  The ECHS  Polyclinic is now being shifted to the newly constructed permanent location wef 21 May 2008.

The new ECHS Polyclinic is located on the other side of Medical Training Centre (MTC) on Cambridge road, opposite to  Sainik Aram Gruha.

In view of the above, the ECHS Polyclinic at CHAFB will be closed on 19 and 20 May 2008.  Only emeregency cases will be attended to at CHAFB MI Room.

Entitlement of Medical Treatment in Service Hospitals

 

Appended below is extract from DGAFMS/DG-3A, Min of Defence letter No. 16307/ECHS Gen/DGAFMS/DG-3A dated 21 Nov 2007 for information all concerned.



“1. Following clarifications are made regarding medical attendance in service hospitals.

2.  ECHS Members

  • Will be attended at ECHS polyclinic and referred to service hospitals/empanelled diagnostic and treatment facilities for special management as per existing orders. The ECHS policy for its members as laid down from time to time would be followed.

3.    Non-ECHS Members

  • Drawing FMA. They are not entitled medical attendance at service hospital vide GOI MOD Letter No. 1(1)/98/D(Pen/Sers) dated 15 Jun 98.

  • Not Drawing FMA. Will continue to avail facilities of medical attendance at service hospitals till further orders at hithertofore.

4. Treatment of Senior Citizens. The following members will be treated as Senior Citizens. They are to be given top priority for all the medical treatment at polyclinics i.e., for Registration, issue of medicines, visiting doctors etc.

  • 75 yrs and above

  • Females : 70 yrs and above

Subject: - PROCEDURE FOR PAYMENT AND REIMBURSEMENT OF MEDICAL EXPENSES UNDER ECHS

Sir,

The procedure for payment and reimbursement of medical expenses under ECHS has been outlined in Ministry of Defence letter No. 24(8)/03/US(WE)/D(Res) dated 19th December 2003. Para 4(b) of the letter ibid says that the rates of payment to empanelled hospitals/diagnostic centers will be governed by the package rates as laid down for CGHS.

2. The Ministry of Health & Family Welfare have reduced the package rates of empanelled hospitals and diagnostic centres for various treatments/diagnostic procedures in December 2006 for Delhi area and June/July 2007 for the rest of the country covered under CGHS. Accordingly the Central Organisation, ECHS also revised the rates for ECHS in consonance with CGHS rates w.e.f. 08th December 2006 for Delhi region and 01st December 2007 for rest of the country.

3. CO, ECHS has been representing, since October, 2007, that consequent upon reduction in rates referred to in Para 2 above, most of the reputed hospitals all over the country have either disassociated themselves from ECHS or are accepting patients of selected specialties like Cardiology and Ophthalmology only. This has resulted in non-availability of appropriate treatment to ECHS patients, which is causing widespread inconvenience and discontentment amongst ex-servicemen. The Chairman of the Chiefs of Staff Committee and Adjutant General have also expressed their concern in the matter and requested to allow ECHS to follow pre-November 2006 CGHS rates.

4. After careful consideration the Government have decided that reimbursement of medical expenses under ECHS to the empanelled hospitals/diagnostic centers would continue at pre-revised CGHS rates as an interim measure.

5. In the meantime a committee of senior officers of the Ministry will go into the whole gamut of issues and prepare future course of action in the matter. Orders for constituting the committee are being issued separately.

6. Cases already settled at revised CGHS rates shall not be reopened

Ex-Servicemen’s Contributory Health Scheme (ECHS)
Origin, Scope and Organisation

As on 08 Feb 08, out of the large number of specialist hospitals available in Bangalore, only the following four are empanelled with ECHS:

  • Nethra Dham - Ophtalmology
  • Narayana Netralaya - Ophthalmology
  • Medinova - Diagnostics
  • HOSMAT  - Hospital for Sports Medicine and Trauma Care

1.     The scheme came into effect on 01 Apr 03 as a public funded Government Health Scheme for provision of medical care to Ex-Servicemen (ESM) in receipt of pension or disability pension and their dependants. It is also applicable to those drawing family/ special family pension as Next of Kin (NOK) of deceased pensioners. It is NOT an insurance scheme and the facilities provided are not linked to the one-time contribution of members. It provides for all types of treatment available in the country and there is no limit to the expenditure that can be incurred on a single member as long as it is essential.

2.     Withdrawal of AFGIS/ AGIF (MBS). The medical Group Insurance schemes of the Army and the Air Force ceased as on 01 Apr 2003. ESM can claim refund of the amount paid towards membership of these schemes by applying to the AGI/AFGI Directorate along with copy of ECHS membership card, AGIF/ AFGIS card and their bank details.

3.     At present ESM pensioners are entitled to treatment at Military Hospitals, not as a matter of right but subject to availability of bed space, doctors and medicines. By joining ECHS, they become authorized category through the scheme.

4.     ECHS is a tri-services project with a central organization headed by Adjutant General, Army HQ assisted by Director General Discipline, Ceremonials and Welfare and Managing Director ECHS.  AF Coordination is with the Director of Personnel Services- AOA’s Branch at Air HQ. Countywide coverage is through 13 Regional Centres and a number of ECHS stations/cells and polyclinics at various locations.

Eligibility

Persons Eligible

5.    The following categories are eligible for membership of ECHS subject to the provisions noted against them:

  • ESM drawing pension /disability pension.
  • Widows drawing family pension.
  • Re-employed officers/PBORS. During the period of re-employment, the ECHS membership card will be kept in suspended animation and members will only avail themselves of service medical facilities. The onus of informing ECHS about re-employment will be on the individual members. Use of both service and ECHS facilities will amount to misuse and will result in termination of membership.
  • Reservists ESM in receipt of pension/family pension of Rs 1275/- pm. Those who have opted for ex- gratia payment are not eligible.
  • DSC personnel. Personnel in receipt of only one pension for having served in DSC are not considered as ESM and hence NOT eligible for ECHS membership. However, if a person having retired from Armed Forces after attaining ESM status and has earned pension there from, joins DSC as a second career, he will be eligible for ECHS membership.Recruits who were boarded out/released on medical grounds AND granted medical/disability pension. Their dependant parents, wife and children are NOT permitted to be included for ECHS benefits. A recruit who was already married prior to joining the training at the Regimental Centre would be permitted to have his wife and children , if any, (not parents), included as his dependants. This would be checked from Part II order published.

Husband and Wife Pensioner/ Govt Employee

6.    If both husband and wife are defence pensioners, only one has to make the ECHS contribution. However both can cover their respective parents as dependants by making two independent contributions. When one is a defence employee and the other a Central Govt employee, the latter can choose to withdraw from CGHS when the spouse joins ECHS.

Eligibility - Dependants

Parents

7.    Father, mother and the ESM pensioner shall be deemed to be dependant, if they normally reside with the ESM pensioner and the combined income from all sources does not exceed Rs 2550/- pm. Parents of the spouse of the ESM are not entitled to ECHS benefits. If a widow in receipt of family pension joins the ECHS, the parents of her late husband can be made dependants, subject to fulfilling the conditions. However, her own parents CANNOT be made dependants.   'Parents i.e. mother and father' of unmarried deceased soldier and in case of deceased parents, the 'Next of Kin' of unmarried deceased soldier are also eligible, provided they are in receipt of liberalised family pension

8.     Spouse. Legally wedded husband/wife including more than one wife. Spouse living separately is included as dependant, as long as the ESM pensioner is responsible for her maintenance. When the spouse is legally separated, he/she cannot be termed as a dependant and hence cannot be made a member of ECHS. In the event of plural marriage, where it is permitted by the Rules, the following conditions should be fulfilled for claiming ECHS membership:

  • Necessary casualty for entering into a plural marriage should have been published through Unit Part II Orders. Names of both wives should be found recorded in the Service Discharge Book/Service Particulars/Retired Officers Booklet issued by respective Service Hq.
  • The names of both the wives should be found recorded in the PPO for grant of 'Family Pension award.
  • In the case of widows, both wives should be in receipt of a share of 'Family Pension' and 'PPO' produced in support of evidence.
  • Daughters .Details must exist in the service record of the pensioner. Unmarried and unemployed daughters and widowed/divorced daughters dependant on and residing with the pensioner can be included as dependants. However, the children of such daughters are not eligible.
  • Son(s) .Unemployed and unmarried son(s) up to the age of 25 are permitted to be included as dependants. Details must be included in the Service Record of the pensioner.
  • Mentally/Handicapped Children. Mentally/Handicapped children who are unable to earn their livelihood are permitted to be dependants for life time provided the onset of disability is prior to attainment of 25 years of age, permanent disability is certified by a Service- classified specialist as per format of the certificate, the condition of disability is mentioned in the records of pensioner and the disabled dependant is eligible for family pension.
  • 9.    A member of any other Govt/PSU health scheme is NOT eligible for ECHS membership unless he cancels the membership of such other scheme

    Membership

    Preliminaries

    10.    Application form. This may be obtained from all Army Station HQ, all AF Stations, Rajya Sainik Boards and ECHS Centres/Stn HQ. It may be also downloaded from Indian Army web-site http://www.indianarmy.nic.in/arechs.htm. The form consists of the main application, format for affidavit, receipt for document, instructions and bank Military Receivable Order (MRO). These need to be checked while collecting the form.

    11    Pay Pension Order (PPO). The latest copy of PPO/ Corrigendum is mandatory for ECHS membership. If not already received, JCDA/CDA concerned must be requested to provide the same. PPO must contain endorsement of spouse’s name for sanction of family pension. If there is no such endorsement, immediate action must be taken to get it done.

    Post 31 Mar 03 Retirees

    12.    Eligible personnel of all ranks retired or retiring on or after 01 Apr 03 will compulsorily become members of ECHS. Such of these eligible personnel who could not submit their ECHS application form to respective service HQ/Record Office prior to the due date of their retirement are permitted to submit the application form to the nearest Station HQ/ ECHS Regional centre on receipt of PPO. They must also furnish a certificate of not having earlier submitted the application / applied for ECHS membership.

    Pre 01 Apr 03 Retirees

    13.    Eligible personnel retired prior to 31 Mar 03 can exercise their option to become members of the scheme latest by 31 Mar 08. After that date, medical facilities will not be provided to non ECHS members at ECHS polyclinics/Armed Forces Hospitals.

    14.    The medical allowance of Rs 100/- pm presently available to ESM pensioners will be forfeited after they become members of ECHS:

    Contribution

    15.    The rates of one- time contribution were revised as below with effect from 01 Apr 04

    17.    The revised rates are applicable only to those who have paid the MROs after 01 Apr 04. If the MRO payment had been made on or prior to 31 Mar 04, the earlier rates will continue to be applicable irrespective of the date of submission of the application. Those who had paid excess as per the earlier rate can apply to the ECHS Cell/Stn HQ for refund by submitting contingent bill in three copies (with revenue stamp affixed if the refund is more than Rs 5000/), MRO copy, ECHS membership receipt copy and Bank certificate showing Basic Pension+ DP+DA duly attested by the bank manager or latest PPO duly attested by the bank manager.

    Exemptions

    18.    The following categories are exempted from payment of contribution while applying for membership.All ESM who are drawing pension from CDA and who retired prior to 01 Jan 96. Min of Def instructions are yet to be issued on procedure for refund of payments already made.

  • War veteran pensioners disabled due to injuries sustained during proclaimed wars or any other encounters and had been accorded such status. Refund of payments already made may be claimed.
  • War widows in receipt of liberalized family pension.
  • 19.    In respect of personnel retiring on or after 01 Apr 03, deductions for their ECHS membership are made at the time of their final settlement and an endorsement to that effect is entered in their PPO. If no such deduction has been made and endorsed in the PPO and the application for membership is submitted subsequently, the applicable contribution will have to be made by the individual.

    Affidavit

    20.    Applicants for ECHS membership are required to make a declaration of their dependants, changes if any and other specified aspects through an affidavit in the format furnished with the application form. It is to be made on stamp paper of Rs10/- and attested by a magistrate or a notary. It is a legal document. Photographs of all beneficiaries or required to be pasted on the affidavit.

    Filling up and Submission of Application Form

    21.    Designated Stn HQ (Army/Navy/Air Force) at various locations are the only authorized collection points for the application. The pensioner is required to personally report to the designated Stn HQ with completed application form along with the following original / photocopies of documents. Dependants are not required to accompany them.

    22.    Detailed instructions are attached along with the application form. These may be studied and followed carefully so as to avoid rejection of the application due to any deficiency. Special care must be taken in respect of the following:

  • Name(s) of dependants must tally with entries in the service documents and records.
  • All columns and details must be correctly filled in.
  • The form must be filled in capitals and by the pensioner only.
  • The form caters for 60 digits including blank spaces in the address. The address may be restricted to a maximum of 60 characters including spaces so as to avoid rejection of the form or wrong abbreviation when the smart card is made.
  • Passport size photographs- not stamp size- of pensioner and each of the dependants must be pasted in the appropriate boxes in the application form and the affidavit. They may be color or black and white but must be recent ones in civil dress, of good quality with sharp clarity and without any stamp, markings or signature on the photograph.
  • The latest PPO/Corrigendum duly attested by Bank/ Treasury from where the pension is being drawn or endorsed by CDA in original is to be shown at the time of submission of the application form. In the case of pre-1986 retirees who are unable to obtain duplicate copy of their PPO the application form may be accepted without a copy of the PPO provided the membership applied for is only for self and spouse. In such cases, a copy each of the. Discharge Book showing the name of the spouse, Pension Book showing details of pension sanctioned and Non- Effective Strength Records Part II (in case of marriage and birth of any child after retirement and the fact is not shown in the DDPO/Banker’s certificate) must be produced along with the application.
  • Discharge Book/Service Certificate.
  • Two passport size photos of the pensioner and each of the dependents who propose to use the ECHS facilities. These will be in addition to those pasted in the application form and affidavit.
  • Original copy of the MRO as proof of payment of contribution (not required in case of direct deduction as endorsed in the PPO and also those exempted from payment of contribution).
  • Affidavit, duly notarized, in original.
  • Photocopy of ESM Identity card. Original to be shown for verification.
  • 23.    All applicants are advised to make and retain photocopies of the application and all the documents before submitting the ECHS application for membership.

    24.    An individual will be deemed to be a member of ECHS and entitled to make use of medical facilities thereof only when his/her documents are verified and accepted at Regional Centre/ Record Office/ Stn HQ and from the date of issue of ‘Receipt of Documents’.

    25.    A photocopy of the ‘Receipt of Documents’ should be taken for future reference. The validity of the receipt for getting ECHS facilities pending issue of smart card is only 60 days, extendable by 15 days by the Stn HQ and by another 15 days by the concerned Regional Centre.

    SMART Card

    Issue/Reissue and Activation

    26.     After due verification and scrutiny of the application, smart cards and add-on cards as applicable will be issued to the applicants through the designated Stn Hq. Applicants are advised to keep in touch with the Stn Hq and, on confirmation of arrival of the smart card, personally report there, along with the original receipt, identity card and demand draft @Rs 90/- per card made out to the appropriate Regional Centre (ECHS), NPF A/C, payable at the city of the Centre, (if not paid with the ECHS application), to collect the smart card. In case of loss of the original receipt, FIR should be filed at the police station and an affidavit for loss prepared on a Rs 10/- non judicial stamp paper. Copy of the FIR and the affidavit in original must be produced in lieu of the lost receipt.

    27.     White Card. ESM whose authorised dependants are permanently disabled is required to possess white coloured smart card. ECHS application form must be submitted with details of ESM and dependant (but photographs of dependant only pasted) along with the old add-on card of the dependant, demand draft for Rs 90/-, three passport size photographs of the dependant and certificate of disability as per specified format to be obtained from the Stn HQ. The disability should be more than 40%.

    28.     Add-on Cards for Parents. . With effect from 01 Feb 06, treatment to parents will be given only on the master card. Add on cards if any held by parents are being withdrawn.

    29.    Updating on Installment Payment. Members paying the subscription in installments need to get their smart cards updated at/ through the issuing office at the time of each installment payment.

    30.    Activation. All cards have to be first activated by recording the fingerprint biometric data at the parent polyclinic and are usable thereafter.

    Procedure in Eventualities Pertaining to Smart Card after Initial Issue

    Corrections/ Amendments

    31    The following points may please be noted:

  • Every member is advised to check the smart card on receipt and report errors and defects if any and return the card immediately to the issuing office along with an application indicating the error and the correction needed, ECHS application form in duplicate with the corrections and three passport size photographs of the ESM and each of the dependants. In case of defect in any of the cards ie master card and add on cards, all the cards are to be returned for correction and reissue. In the event of any subsequent amendments sought by the card holder or damage to the card, the applicant has to quote the original smart card number and pay the required amount by demand draft at the prevailing rate as done for the initial card.
  • Add-on Card for Dependant. ECHS application form to be submitted in duplicate with details of ESM and dependant(s) along with three passport size photographs of dependant(s), and demand draft of Rs 90/- per card.
  • Inclusion of Dependant(s) in Master Card. ECHS application form to be submitted in duplicate with details of ESM and all dependants including the new ones, along with the Master Card already issued, four passport size photographs of ESM and each of the dependants, demand draft for Rs 90/- for preparation of the new Master Card, affidavit including ESM and the new dependant(s) and Part II order/POR copy/Dependant certificate from the Record Office.
  • Loss of Master ECHS Card/Add-on Card under Circumstances beyond the Control of the Individual. FIR must be filed at police station and an affidavit of loss prepared on a Rs 10/- non-judicial stamp paper and duly notarised. ECHS application form must be submitted, less MRO, with details of ESM all dependants filled in and photographs of ESM and all dependants pasted (photographs of only dependants in case of loss of add-on card), along with copy of the FIR, affidavit for loss in original, photocopy of the ECHS card(s), demand draft for Rs 90/-each for preparation of new card(s) and three passport size photographs of ESM and each of the dependants (photographs of the dependants only in case of loss of add-on card). A member can be issued a maximum of one/ one set of duplicate smart card. In case of loss of card for the second time the membership would be terminated.
  • Use of Smart Card on the Demise of ESM Pensioner

    32.    In the event of the demise of the primary card holder (ESM Pensioner), the remaining eligible dependents are entitled to avail themselves of ECHS Facilities on production of the master card/ add on card issued earlier. In such cases Stn Hq should be contacted to initiate further action.

    Care and Maintenance of Smart Cards

    33.    Retain photocopies of receipts and smart cards for ready reference. Do not laminate the cards. Keep them securely and free from dust and dampness at all times.

    Treatment – Eligibility Dependants

    Polyclinics

    34.    The polyclinic is the nodal point where the patient has to report in the first instance for availing himself/herself of any kind of treatment. A total of 227 polyclinics are established across the length and breadth of the country.

    35.    Parent Polyclinic. Every member is assigned to a ‘Parent Polyclinic’, generally the one in his/her home/affiliated district, where he/ she is normally expected to report for any requirement. A member may request for assignment to a different polyclinic provided it is nearer to his/her permanent address than the one in his home/affiliated district.

    36.    Treatment at the parent polyclinic will cover the complete range of facilities established therein. A member patient may also be treated at any of the ECHS polyclinics anywhere in the country, including referral to an empanelled facility in an emergency/essential situation, if such need arises. But the full range of facilities and referral under normal circumstances will be available only in the parent polyclinic. Medical equipment for home use will be sanctioned only by the parent polyclinic.

    37.    Change of Parent Polyclinic. Members desiring to change address/polyclinic dependency are to submit application with an affidavit to the Stn HQ nearest to the new address. The old smart card is to be cut into two pieces and submitted along with a deposit of Rs 90/- in favour of the appropriate Regional Centre for issue of a fresh card.

    Dental Treatment

    38    Dental treatment available through ECHS will be limited to specified procedures. Prosthodontic clinical work will be carried out at ECHS polyclinic only. However laboratory work and prosthodontic procedures, including clinical work, of fixed partial denture and acrylic denture of one jaw or both jaws may be outsourced to empanelled dental laboratories/facilities.

    39.    In the event of non-availability of Dental officer at Polyclinic, patients may be referred to Service Dental facilities for prosthodontic work. Such referral will be on appointment only and subject to availability of facilities in the concerned Dental Centres.

    40.    Referrals for specialised dental services will only be made after the facilities in the ECHS Polyclinics are utilized fully. When a referral is made, the Dental officer making the referral will clearly state the diagnosis and the procedures/treatment required on the Referral Form. Referrals for lab work will be made to facilities specifically empanelled for laboratory work.

    41.    Dental procedures relating orthodontic treatment, all cosmetic rehabilitation including dental Implants and related/subsequent prosthetic work and removable metal (cast) partial dentures are NOT included in ECHS and will NOT be made available from polyclinic/service dental centres/empanelled facilities.

    42.    Treatment of Senior Citizens. Patients above 75 years of age will be treated as senior citizens in the ECHS polyclinics and will be given priority. They will be attended out of turn at reception, examination by doctors and issue of medicines.

    Empanelled Hospitals/Facilities

    43.    A large number of hospitals/facilities throughout the country are recognized and empanelled by ECHS so as to augment, where necessary, the facilities available at the polyclinics and Service Hospitals. Case by case, the empanelment process gets completed with the signing of the Memorandum of Association with the hospital concerned and thereafter only does the facility become available.

    Referral

    44.    If, after all available facilities of the polyclinic are fully utilized, the required diagnostic/ treatment facilities cannot be provided by the polyclinic, the patient will be referred to the local Service Hospital. Only in case of non availability of treatment facility at a Service Hospital for any reason, referral will be made by the polyclinic to an empanelled hospital/facility of the patient’s choice.

    45.    All referrals for treatment /diagnostic procedure at empanelled facilities are to be generated from the polyclinic. The patient has to report to the empanelled facility with the smart card and a referral form issued by the polyclinic

    46.    After discharge from the empanelled hospital/ facility, the patient will report to the polyclinic for review by the ECHS MO/Specialist for advice /medicines

    47.    Follow up Treatments or Reviews. In case regular follow up treatments or reviews are required, such follow up treatments or reviews (OPD/ indoors) will be provided for a maximum period of one month. Referral forms in such cases should mention the same. Fresh referral is required on the expiry of the one month period

    Payment of Bills

    48. Following are rates of payment:

  • On completion of treatment/ other procedure, the patient is required to sign the bills raised by the referral hospital. The bills for agreed treatment/ procedures are paid directly to the empanelled hospital by ECHS at rates agreed upon. The patient is NOT required to pay.
  • Certain facilities like telephone, TV, transport, food/catering (except in some specified cases) are not entitled and charges for these have to be borne by the patient if availed of.
  • Certain available procedures require prior approval as per laid down guidelines. Unapproved procedures, if carried out, should be with the patient’s consent. Payment for such procedure has to be borne by the patient and is not liable to be made by ECHS.
  • Payments made by ECHS members to empanelled hospitals/ facilities are NOT reimbursable.
  • Medical Advances for ECHS Members. Certain hospitals of national repute and Government hospitals may demand advance payment from patients for treatment to be provided. ECHS members desirous of taking treatment in such hospitals have to obtain referral from the polyclinic. Information on such hospitals and procedure for and extent of medical advance available to members can be obtained from the polyclinic.
  • Emergency Treatment

    49.    In case of emergency, an ECHS beneficiary can get treatment at any of the following:

  • Service Hospital. Free treatment, no further action is required.
  • Empanelled Hospitals. If directly going to an empanelled facility, the member has to produce the ECHS card as proof of membership. In such circumstances the empanelled hospital is required to inform the polyclinic of the station or in case the polyclinic cannot be contacted, the nearest service hospital/ Stn Hq, within 48 hours. A referral is then generated by the polyclinic for the hospital. Free treatment is provided at approved rates.
  • Non-Empanelled Hospitals. Nearest polyclinic is to be informed within 48 hrs and an emergency information report collected from the polyclinic. Bills are to be paid by the patient. Claim for re-imbursement is to be submitted to parent polyclinic within one month of termination of hospitalization. Reimbursement will be allowed at approved rates.
  • Onus of proof of emergency lies with the patient.
  • 50.    Generally, applicable conditions of emergency are:

    • Acute cardiac conditions /syndromes
    • Vascular catastrophies, Cerbo-vascular accidents
    • Acute respiratory emergencies
    • Acute abdomen including acute obstetrical/gynecological emergencies
    • Life threatening injuries
    • Acute poisoning and snake bite
    • Acute endocrine emergencies
    • Heat stroke and cold injuries of a life threatening nature
    • Acute renal failure
    • Severe infections leading to life threatening situations
    • Any other condition in which delay could result in loss of life or limb.

    Medicines

    Issue from Polyclinic Dispensary

    51.    Please follow the guidelines mentioned below:

  • Medicines will normally be issued by the polyclinic for duration as prescribed by the treating doctors. Such prescribed medicines will, by and large, be available from the polyclinic dispensary. Medicines not available will be prescribed only if absolutely essential and unavoidable.
  • On the expiry of the duration prescribed by the treating doctor, fresh prescription will be normally required for further issue of medicines. PVMS and NIV medicines from ECHS polyclinic may be issued for a maximum period of 90 days for chronic ailments of hypertension, IHD, diabetes mellitus, tuberculosis, epileptic disorders or any other chronic diseases deemed fit by SEMO. This is subject to the medicine having been prescribed by the concerned specialist for the required duration and the patient not being required to be reviewed during the period of prescription.
  • Issue from Polyclinic other than the Parent Polyclinic

    52.    Please follow the guidelines mentioned below:

    • Issue of medicines from a polyclinic other than the parent polyclinic will be restricted to a maximum of seven days at a time. Medicines for further treatment are to be collected from the parent polyclinic. In case the patient needs medicines for a longer period from the non- parent polyclinic, he/ she may obtain a temporary transfer/ attachment form from the parent polyclinic on production of which the non- parent polyclinic may issue medicines during the specified period.

    53.    Issue from Service Hospitals. For patients treated / consultation given in any Service Hospital, medicines will be issued from the same hospital. However, if long term treatment is required, the patient will get medicines for use beyond one month from the parent polyclinic. For this, the patient should collect medicines for up to one month from the Service Hospital concerned and then, as soon as possible, put in a demand with the parent polyclinic for further treatment period.

    54.    Approved List of Drugs. Medicines issued from polyclinics will be limited to an existing list containing all essential drugs as approved by DGAFMS and periodically updated. If a particular drug is not mentioned in the list, the equivalent/ alternative generic drug will be issued/ demanded. Medicines prescribed on discharge from an empanelled hospital will be issued from the polyclinic. In case of non-availability, the polyclinic will take necessary action for procurement/ local purchase as per laid down procedures.

    55.    Brands. Sometimes brands of a particular medicine issued may be different from that prescribed by an empanelled hospital but ECHS staff will ensure that the chemical composition/ therapeutic efficiency would be identical.

    56.    Provision of ECHS Drugs at Local AF Stn Medical Centre. In Bangalore, ECHS drugs will be positioned at AF Stn Jalahalli and HQTC (U) based on certain guidelines. Only routine drugs such as anti hyper-sensitives, anti-diabetics, cardiac drugs, analgesics, and anti-biotics will be so provided. No costly drugs such as anti-cancer drugs or consumables will be provided except in exceptional situations on a case to case basis after obtaining written approval by competent authority (PMO Command).

    57.     Reimbursement of Cost of Medicines. As a rule, reimbursement of cost of medicines bought on own initiative by a patient is NOT permissible. For anti-cancer and other specified categories of drugs, the patient will submit a demand to the polyclinic immediately on discharge from the hospital. If the same is not available from the polyclinic, the patient can purchase the medicines for one month period and claim reimbursement of the same. This is permitted only in cases of post operative major cardiac surgery / intervention cardiology, oncology, post operative organ transplant post operative joint replacement and post operative major neuro-surgical / neurology.

    58.    Reimbursement of Medical Expenses after Demise of ESM/ ECHS Member In the event of the demise of an ESM/ECHS member before permissible reimbursement of medical expenses and cost of medicines could be credited to his/her account; the claimant has to submit the following documents to the ECHS polyclinic for further processing:

  • An affidavit on stamp paper of value not less than Rs 10/-duly notarized. Specimen copy of the affidavit may be obtained from the designated Stn Hq.
  • A "No Objection" certificate from other legal heirs, if any, on similar stamp paper. Specimen copy of the certificate may be obtained from the Stn Hq.
  • A copy of the death certificate and smart cards of the ECHS Member
  • Hearing Aids

    59.    Hearing aids will have to be recommended only by a Service ENT specialist (two specialists in case of digital hearing aid). Officer i/c polyclinic will obtain the endorsement of the recommendation by Senior Adviser/ Additional Senior adviser of local station/ zone and forward the case to ECHS Central Organisation for approval. After such approval he will Issue of ’Authorisation of Hearing Aid’

    Suggestions/Queries/Complaints

    60.    Suggestions, queries and complaints relating to local issues which are addressed directly to the central organization are eventually investigated by the concerned polyclinic/ Stn HQ/ Regional centres/ Command HQ. In most cases the issues could be resolved at the local level in the first instance itself thus obviating the delay involved in the process of reference between different levels. Such suggestions/queries/ complaints may therefore be addressed directly to the concerned polyclinic/Stn HQ/ Regional Centre. However matters relating to policies may be addressed directly to the Central Organisation.

    ECHS Polyclinic – Command Hospital Air Force, Bangalore

    Introduction

    62.    CHAF is one of the largest multi-speciality service hospitals in the country. It is equipped to provide quality health care and state-of-the-art super speciality services to every patient. It is also a leading Post Graduate Medical Training Institute.

    • For ECHS members attached to the polyclinics in Bangalore and Karnataka, CHAF is the main referral service hospital for facilities beyond those available in the polyclinics.
    • The wide range of specialized diagnostic, laboratory testing, surgical and intensive treatment facilities established in the hospital includes a state of the art MRI Scan Machine, Cardiac Catheterisation Laboratory, Special Care Unit (SCU) for management of patients who no more require ICU care but are acutely ill and hence not fit to be admitted in general wards and a de-addiction centre.
    • By and large, CHAF can handle almost all cases of ECHS members referred to it and only in exceptional circumstances referral to other empanelled hospitals may be necessitated.

    Sick Report Procedure

    66.    The Sick Report is as follows:

    • Always carry the original Smart Card/Regn Slip and produce the same at the Reception on demand.
    • Have a small note book for each patient with all the details of the patient written and a copy of the Smart Smart Card/ Regn Slip pasted on the inside cover.
    • Hand over the note book to the Reception. For PBORs the book will be returned to the patient along with a numbered token.
    • For officers, the book will be sent directly to the doctor.
    • PBORs will be seen by the doctor as per their token number. Officers will be called separately on the PA system as per the register.

    Issue of CT Medicines

    67. The procedure is as follows:

    • Hand over the note book to the Reception counter for writing the prescription chits.
    • Get the prescription chits signed by any one of the doctors of the Polyclinic.
    • Collect the medicine from the Dispensary.
    • In case of non-availability of the medicine, the patient will be given a separate chit for collection of the medicine from the CHAF Dispensary or for raising local purchase proforma.
    • Before leaving, please check and confirm that the medicine issued is strictly as per the prescription given.
